I hadn't yet added a shipping service when I posted the question. How do you enable the shipping address fields during checkout? On my site the only address that appears during checkout is the Billing Address. I did check the option in the control panel to allow customers to ship orders to an address other than the billing address, but I still don't get the option to input a different address.
