Figured it out!

Go to file OnePageCheckout.asp

Remove

style="display:none

from

<table class="pcShowContent" id="nickname" style="display:none; border: 1px #eee solid; margin-bottom: 10px;">

If you want to expand comments by default you can also.

remove

style="display:none

from

<table class="pcShowContent" id="comments" border: 1px #eee solid; margin-bottom: 10px;">

We receive a lot of orders where it would be normal for our customers to  name their orders. They don't seem to be doing this since we upgraded to ver 4 apparently because it is listed as optional in the checkout process and they don't bother to click and explode the option so they don't name their orders. An order name would be very useful to us internally as we process a lot of wholesale custom repeat orders. 

In our previous version the order nickname field was always apparent during checkout and customers typically nicknamed orders. Is there a quick and easy tweak to the code to have that nickname order "option" always selected or even required maybe set a 0 to a 1 for some setting on a page.
