Our categories have been a bit slow for a while (off and on), and all of a sudden we are suddenly experiencing HUGE slowdowns for customers clicking on our categories.

The categories dbo was 87.5% fragmented. We re-indexed it and 5 minutes later, it was already 75% fragmented again. Anyone experienced this, to have an idea what might be causing it?

Everyone says "Nope, I haven't made any changes to how we handle categories or db queries recently".
