I have a customer who lives in the Philippines but ships the items to her sister who lives in California. It looks like because of her billing address her shipping options are cut down to "Other Shipping Options" and USPS. Everything else isn't shown, and we also ship using FedEx and UPS. If she sets the shipping address to her billing she gets international for UPS, FedEx, USPS and Other Shipping Options, if she sets her billing as her sisters she gets all the options only domestic. The problems only occur if she using her out of country billing address to ship domestically.

I was able to get the same results on the demo cart on this site. Is this a bug or by design?
