If you want the mesage only to appear to customers in a specific location then you need to custom code it.
 
The easiest way is to just add a general message on the page so that everyone can see it and if they are an international customer they can click on it.
 
It's no different than a message that says 'free shipping over £x' as that applies to a specific group of customers as well.

You could try the following...

Retireve the visitor's IP address.
Pass it to a geo ip locating service and use the returned data, most likely country to determine whether to show a message or not. I found one, freegeoip.net but have never used it so am not endorsing it, just a lead to follow up on. Alternatively there are free ip databases you can use if you are in the mood to have some real fun... You can use the info to make your own in house geolocation DB, if you prefer...

You could code it so any country code other than say "US" will pop up a modal window/dialog box with the appropriate message which the visitor will need to close before continuing.

Hello. Is there anyway to add a message to the checkout for users of a specific country? I want to add a message to all our international customers about shipping. I know I could make the message a custom page and just have a link to it, but I want it prominent so they click on it.
