Recently, I've had a few clients whose business rules require some form of a wholesaler application form - much like the affiliate application form and/or like the "Create Account" form; usually a combination of the two with custom business rules applied.

So far I've got as many variations on this theme as merchant who need it.

I'm interested to see how many other merchants might like to see such an add-on and what business rules they'd require.
