For some reason, the icons for the buttons and required fields are all missing on my pages???

I know - everything is breaking out of my frame...we're working on that.  But, at the bottom - there should be an image there - I'm assuming a button.  When I view the source, it just says <img src="">.  Any ideas???  I'm lost.  Thanks.
