Here's a clue:
 
When I try to navigate directly to one of the captcha images (http://www.lakesmedicalequipment.com/store/pc/img.asp?p=1&a=6&b=10/2/2009 12:41:17 PM), an error message is returned:

Hi,
  It's nothing to do with FreeType, I've not heard of that before. The usual cause is the XML Parser, or sometimes, I think, folder permissions. You can check the XML parser selected via your admin pages Help > Troubleshooting Utility, I V3 is available I recommend selecting that one. The folder permissions are covered in the WIKI (See the link at the top of the forum page). If neither resolve it please raise a support ticket.

States that XML Parser is installed on the server.  I read somewhere that FreeType needs to be enabled on server.  Is this needed as well as a XML Parser?
